Payment and Billing
- Payment is expected at time of service
- We accept checks, Visa, Mastercard and American Express
- When payment is received a receipt with coding for insurance reimbursement will be issued to you.
- We do not bill any insurance companies.
- We will fill out treatment reports needed for reimbursement.
Scheduling and Appointments
- All appointments are scheduled through my assistant Carol
- Patients must be seen a minimum of every 3 months
- After school appointments are scarce; please book in advance
- Appointments cancelled with less than 24 hours notice are subject to full charges
Prescription Refills
- The office requires 1 full week notice for all refill requests.
- Please minimize same day or next day requests - it is unlikely that they will be filled in a timely manner.
Telephone and Voice Mail
- Carol is usually available between 12-3 pm
- Messages left after 3 pm will be returned the next business day
